U.S. Joint Performance Appraisal Form & Process
The U.S. Joint-Appraisal process is designed to provide both the employee and the employer an opportunity to communicate their individual perception of the employee’s performance as well as the performance of the employer.
This is an opportunity to contribute significantly to improving both your own performance and your working relationship with your manager.

This process is intended to:
Open a dialog
- With regard to area’s where the employee may need to improve or where the performance is acceptable or possibly even above expectations
- With regard to area’s where the employee feels the employer should improve or communicate needs that relate to their ability to perform or improve their performance
- Document key aspects of performance both past and in terms of future expectations
As part of a performance appraisal, any previous appraisals should be referenced and reviewed
- Was action taken with regard to previous appraisal results?
- Has performance improved, declined, or stayed the same since the last appraisal?
The Process
- Employee completes their section of the appraisal form and submit it to the employer
- A joint appraisal meeting is held to review and discuss the results
- Employer completes their section of the appraisal form and returns it to the employee
No Surprises
One of the key goals of the above process is to open candid dialog and to ensure there are no surprises contained within the final appraisal.
The self-appraisal segment of this process encourages you to be proud of your accomplishments and candid about your areas of improvement. You should also communicate any problems that get in the way of performing your job and articulate any specific needs you have that would help in the performance of your work.
The following questions, therefore, are intended to help you organize your thoughts and share information with your manager prior to receiving your performance appraisal. An essential goal of the performance appraisal meeting is that both you and your supervisor know clearly what you expect of each other and feel strongly that you can achieve your objectives by working together.
